NCCC Founders Day 2025 Community Service

THE JENNIFER M. JONES FOUNDATION

Dr. Jennifer M. Jones Foundation (Legacy Project) a 501c3 Nonprofit organization that provides intensive case management services.

We partner with members to provide advocacy, individualized plans to better their lives, support accessing health care and guidance through life transitions, such as housing insecurity, pregnancy and parenting, navigating life after release from incarceration, mental health, substance use disorders and much more, for youth and adults.

We provide a variety of workshops and outreach to promote healthy communities.  Our project connects communities to resources necessary for long-term success.  We help others reach their ideal finish line.

Our staff will meet you at a shelter for the unhoused, an encampment, in the hospital, doctor’s office, board room, a park, or the community center.  We help bridge the gap between a troubled today and a successful tomorrow………..We are “A Hand Up, To Be, A Hand Up,”……..we help you, you help someone else.
